Assisted living
Assisted Living is for people aged 65 years and older who live at home in the Hawkesbury, L’Orignal and Vankleek Hill area. This service is provided by Personal Support Workers in collaboration with Groupe Action.
This program is intended for individuals who require a high level of personal support on a daily basis. However, they do not require the supervision or medical care that would be provided in a long-term care home.
We coordinate all the care and services you need to continue living at home.
Services vary depending on your personal needs and may include a combination of:
- Personal support: getting in and out of bed, dressing, bathing, taking medications and other daily activities
- Domestic assistance: meal preparation, cleaning, laundry, shopping and other activities of daily living
- Safety: personal assistance system, visits and safety checks
How does the Assisted Living program work?
We coordinate all the care and services you need to continue living at home.
How to access this service?
You must first contact Champlain Home and Community Care Services . They will assess your situation and send us a referral if you qualify. You can reach them at 310-2222. You must have a valid Ontario Health Card.
Is there a fee?
There is no charge for this service if you qualify for the program
